Canadian Livestock Records has provided an Electronic Application Form
for your use. You have the option to order your certificate via the Internet
or you may print out the Application Form and mail it to this address.
La Société Canadienne d’Enregistrement des Animaux vous a fourni une
demande d’enregistrement électronique. Vous avez l’option de commander votre
certificat via l’Internet ou vous pouvez imprimer la demande et l’envoyer
par courier à

.. IF you are ordering your certificate (with photo) .... as we need a photo..
.. or either parent is registered ... as we need the parents certificate..
.. or the animal has no permanent identification (tattoo,
ear tag, micro chip) submit a colour photograph (maximum 4" x 6" landscape
format) showing the entire animal and any distinctive markings. (Photo fee applies)
A cheque or credit card authorization must accompany this application.

.. ou si l’animal n’a pas d’identification permanente (tatouage, étiquette d’oreille, implant électronique), soumettez une photo couleur (maximum 4”X6” format paysage) qui montre l’animal en entier et toutes marques distinctives. (Frais de certificat avec photo s’applique).
Une chèque ou une autorisation de carte de crédit doit accompagner cette demande.
